What this site does
This site asks a few questions and, if you agree, passes your answers to Family First Life and its licensed agents so they can contact you about life insurance. Life Plan Review is not an insurance company. Submitting the form does not buy a policy, and it does not promise coverage, a price or approval.
Who can use it
- You must be at least 18 years old and live in the United States.
- The offer is not available in every state. If the quiz tells you it is not available in yours, we cannot take your request.
Your information
- Give accurate details that belong to you.
- When you tick the consent box and press Submit, you agree that Family First Life and its licensed agents may contact you at the number you gave, as described in the consent wording shown on the form.
- Consent is not a condition of purchase.
- See our privacy policy for what we collect and how to opt out. You can reply STOP to any text message at any time.
Using the site
- Do not submit details for someone else without their permission.
- Do not use bots or scripts to fill in the form, and do not try to disrupt or break into the site.
Limits
The site is provided as is. We do not promise it will always be available or error free. To the extent the law allows, we are not responsible for losses that come from using it.
Changes
If these terms change, the new version will replace this page.
