Privacy policy

Life Plan Review runs this website. It asks a few questions and, if you agree, passes your answers to people who can talk with you about life insurance. This page explains what we collect, who receives it and how to stop contact.

What we collect

Your answers: the kind of work you do, whether you served in the U.S. military, your age and your state.
Your contact details: first and last name, phone number and email address.
Your consent: the exact wording you agreed to, its version, and the date and time you agreed.
Technical details: your IP address, your browser and device type, and the address of the page you submitted from.
How you found us: details carried in the link you clicked, such as campaign and ad identifiers and Facebook or Google click identifiers, plus the Facebook browser cookies (_fbc and _fbp) if your browser has them.
Spam checks: the form uses Cloudflare Turnstile to tell people from bots. It runs in your browser, and Cloudflare's own privacy policy applies to it.
Proof of consent: on some versions of this page we use TrustedForm, a service from ActiveProspect, to keep a certificate showing how the form was completed.

How we use it

To pass your request to a licensed agent so they can contact you about life insurance.
To keep a record that you agreed to be contacted.
To block spam, duplicates and misuse.
To see which ads and pages bring in requests.
To meet legal obligations.

Who receives it

We share your information with Family First Life and its licensed agents so they can contact you about life insurance. They may call or text you, including with automated technology, as described in the consent you agreed to when you submitted the form.

We also use service providers to run this website and to store and route requests. They handle your information only to provide those services.

Stopping contact

Reply STOP to any text message you receive.
Tell the agent who calls you that you want to opt out.
Email [OPT-OUT EMAIL ADDRESS] with your name and phone number, and we will pass your request to the agents who hold your information.

To ask what we hold about you, or to ask us to delete it, use the email address above. We keep information only as long as we need it for the purposes listed on this page or to meet legal requirements.

Who this site is for

This site is for adults aged 18 and over. We do not knowingly collect information from anyone younger.

If this policy changes, the new version will replace this page.
